Job Description:

Prosperity Workforce Solutions is seeking an Occupational Therapist (Case Manager) to join an ABI Outreach Team within a community-based healthcare organization in Scarborough, Ontario.

This full-time, permanent role supports individuals living with Acquired Brain Injury (ABI) and their families across home, hospital, and community settings.

You will work as part of an interdisciplinary team, delivering client-centered care to individuals with complex physical, cognitive, and mental health needs.

Position Details

  • Employment Type: Full-time, permanent (35 hours/week)
  • Compensation: $41.98 – $48.98/hour
  • Unionized environment with comprehensive benefits and pension
  • Standard hours with flexibility for occasional evenings/weekends

Key Responsibilities

  • Conduct client visits in homes, hospitals, hostels, and community settings
  • Complete assessments and develop individualized care and treatment plans
  • Apply health promotion and disease prevention approaches to care
  • Collaborate with an interdisciplinary team and community partners
  • Develop and evaluate client education materials
  • Advocate for client needs across healthcare and community systems
  • Provide consultation and support to internal teams and external partners
  • Participate in care planning, data collection, and performance tracking
  • Utilize electronic health records (EHR) and shared care plans

Qualifications

  • Registered (or eligible) with the College of Occupational Therapists of Ontario
  • Minimum 2 years of community-based OT experience preferred
  • Experience with Acquired Brain Injury (ABI) and/or mental health populations considered an asset
  • Strong understanding of community healthcare systems preferred
  • ADP Authorizer designation considered an asset
